Public Organisations

Declaration by the applicant (DEC) : the file is: Call2011_Promo_P_Annex_Declaration_by_the_applicant. The DEC file must be filled in in all its part, it must be dated, signed and stamped by an authorised representative of the organisation and it must be printed on LETTERHEAD of the organisation. All participant must send the scanned file (in *.pdf format);

Legal entity form (LE): the file is: legEnt_public_xx.pdf. The instructions for the filling are within the atteched file. All participant must send the scanned file (in *.pdf format).

Legal document (LD): a certified copy of the resolution, law, decree or decision establishing the body in question, or if not available, any other official document originating from this body (an invoice for example) or a certified copy of a printed document provided by the city/municipality is sufficient if the header contains the name, address and when applicable the registration number. In the first page all participant must be write “CORRESPONDING TO THE ORIGINAL “ and this part must be signed, stamped and dated. All participant must send the scanned file (in *.pdf format).

Legal document (LD): provide a certified copy of legal documents that prove the legal existence of the organisation and that the organisation is established according to the respective national law(s). Legal documents should be coherent with the legal status on Form A2 of the participant data. If the applicable national law requires a registration, a copy of this registration should be included as well. Legal documents can be submitted in national language. Documents should be signed on the first page by stating on the document “CORRESPONDING TO THE ORIGINAL “ and having this statement signed, stamped and dated. All participants must send the scanned file (in *.pdf format).

Balance sheet (BS) and Profit and Loss account: All participants should provide their balance sheets for the most recent yeat that accounts aree closed. Organisations which according to their national law do not need to establish balance sheet should attach the closest equivalent, e.g. Statement of income and expenditures. Documents should be signed on the first page by stating on the document “CORRESPONDING TO THE ORIGINAL “ and having this statement signed, stamped and dated. All participants must send the scanned file (in *.pdf format).

Simplified financial statement (SFS) : The file : Call2011_Promo_ P_Annex_Simplified_financial_statement.xls. All participants must send the excel file *.xls

FOR Coordinator ONLY:

Audit ReportIn case a participant request EU funding of more than €500.000 an audit report produced by an approved external auditor must be submitted. That report must certify the accounts for the last financial year available and can be submitted in national language. Documents should be signed on the first page by stating on the document “CORRESPONDING TO THE ORIGINAL “ and having this statement signed, stamped and dated. FUNITG must send the scanned file (in *.pdf format).
Financial identification form (file: fich_sign_ba_gb_xx.pdf)The financial identification form needs to be filled out by the coordinator following the instructions.
